Athletic Registration

Athletic Registration

Athletic Registration is a two part process:

 

Part 1


All athletes need a completed physical on the SVUSD physical form. This is good for one calendar year. All incoming 9th graders must complete a physical before they try out for a team; this includes all teams who have summer try-outs. Provide a copy to the coach as the physicals handed to coaches in the summer do not come to the Athletics Office.

Part 2


Once your son or daughter makes a team please register your athlete on www.AthleticClearance.com.

 

Once you register please print a copy of the confirmation form and bring to the Athletics Office in ASB along with a copy of the SVUSD physical form from Part 1. Once your student clears for your designated sport you will receive an email confirmation from the Athletics Office. We will not clear athletes until rosters have been established by coaches and grades have been inputted by teachers. Fall athletes will be cleared in August, Winter athletes will be cleared in October and Spring athletes will be cleared in February.  

Please note: you must register your student for each sport played; if your son or daughter intends to play three sports (fall, winter, spring sport) you will turn in three confirmation forms.

Other Athletic Forms

Driver FormMust be completed to drive student athletes to and from athletic events including tournaments. Driver forms are good for one school year. The driver form must be turned in to the ASB office with a copy of vehicle registration, license, and insurance (including limits).

Drug Testing Information Flyer- Information from SVUSD in regards to enrolling your son or daughter in our volunteer drug testing enrollment program. . 

 

Volunteer Drug Enrollment Form- Please complete this form and give to Brian Cox, Assistant Principal who oversees this program. 

Athletic Transfers- CIF

If you are a transfer from another high school to Simi Valley High School you will need to contact the Athletic Director to complete paperwork to file as a CIF transfer. 

 

Students who are CIF transfers may not participate in any SVHS athletic programs until cleared by CIF.

Athletic Transfers- Sport to Sport

This section is for students who are intending to participate in multiple sports at Simi Valley High School. At the end of your season of sport you must transfer to the next season of sport with head coaches and the Athletic Directors signature. 

 

The Transfer form is found in the Counseling Office. The counseling office will adjust your athletic period once the appropairte signatures are obtained. 

 

Example: Student participates in Cross Country in the Fall and Soccer in the Winter. Once the Cross Country season is over the student will transfer to the Soccer class. The student will go to the counseling office and request the golden rod transfer form. The student will obtain the Cross Country coaches signature, the Soccer coaches signature and the Athletic Director's signature. Once all signatures are obtained the student will walk the form to the counseling office for the class switch. The counselor will switch the class which will show on the students Aeries account.
